As intervenções do enfermeiro de saúde familiar à pessoa com Alzheimer: perspetivas das famílias

Abstract

Nos dias de hoje focar-nos em cuidados de saúde de qualidade exige considerar a família como uma unidade de cuidados. Quando a família se confronta com um ente querido doente, é necessária uma reorientação das suas vidas. Embora se reconheça que a família tem uma grande capacidade de se adaptar face às mudanças sociais, políticas, culturais, entre outras, a situação de doença é um acontecimento potencialmente gerador de crise para a família. Neste sentido, o enfermeiro Especialista em Enfermagem Comunitária na área de Enfermagem de Saúde Familiar é aquele que garante cuidados em parceria, capacitando-a a partir do seu potencial de saúde. Assim, com o objetivo de desenvolver competências especificas e comuns, optou-se pela realização do Estágio de Natureza Profissional numa unidade de saúde familiar da região Norte. Sublinha-se, que no domínio do desenvolvimento de competências específicas, a prática clínica assentou no cumprimento das competências definidas pela Ordem dos Enfermeiros, estabelecidas no Regulamento das Competências Específicas do Enfermeiro Especialista em Saúde Familiar, publicado no Diário da República II Série, nº 135 de 16 de julho de 2018, nomeadamente: cuidar da família enquanto unidade de cuidados, e de cada um dos seus membros ao longo do ciclo vital e aos diferentes níveis de prevenção e liderar e colaborar em processos de intervenção, no âmbito da enfermagem de saúde familiar. Desenvolveu-se também, em resposta às necessidades formativas identificadas na equipa, um manual de apoio ao cuidador da pessoa com Alzheimer, promovendo assim, a melhoria de qualidade contínua. A nível da gestão procurou-se deter um conhecimento aprofundado da organização e funcionamento da unidade de saúde, da equipa de saúde, de forma a adaptar e otimizar os recursos disponíveis de forma segura e com qualidade. No desenvolvimento destas competências, mobilizamos estratégias metodológicas ativas, participativas com recurso a evidências científicas. O estudo de investigação debruçou-se sobre a temática: As perspetivas das famílias da pessoa com Alzheimer face às intervenções do enfermeiro em saúde familiar, com o objetivo de conhecer as perspetivas das famílias da pessoa com Alzheimer face às intervenções do enfermeiro em saúde familiar e com a finalidade de contribuir para a mudança/inovação das intervenções do Enfermeiro Especialista em Saúde Familiar junto da família com pessoa com Alzheimer produzindo ganhos em Saúde. Salienta-se que a demência é considerada como uma síndrome clínica caracterizada pelo declínio cognitivo com caráter progressivo e permanente, causada por múltiplas etiologias, conduzindo a repercussões sociais e ocupacionais na pessoa doente. A doença de Alzheimer, é a demência diagnosticada mais comum e tem repercussões profundas na dinâmica familiar. Utilizou-se uma metodologia qualitativa, um estudo exploratório e descritivo, cuja recolha de dados envolveu a realização de entrevista semiestruturada a familiares da pessoa com Alzheimer no domicílio inscritos numa unidade de saúde familiar da região norte de Portugal. Realizou-se análise de conteúdo aos achados e respeitou-se todos os procedimentos éticos e morais. Verificou-se que a responsabilidade de cuidar de um familiar com Alzheimer vai muito além da prestação de cuidados básicos, implicando uma reestruturação completa da vida dos cuidadores e exigindo adaptações contínuas às necessidades crescentes da pessoa cuidada. A necessidade de conciliar os cuidados com outras responsabilidades pessoais e profissionais, leva frequentemente, a um desgaste significativo, resultando em sentimentos de isolamento, frustração e, muitas vezes, exaustão. Neste sentido, o enfermeiro de saúde familiar deve enfatizar os recursos que mantêm a família em funcionamento de acordo com a sua fase do ciclo vital, estabelecendo uma relação terapêutica com a família, ajudando-a, a encontrar estratégias para gerir com a situação clínica, atenuando a exaustão dos familiares cuidadores.

Nowadays, focusing on quality healthcare requires considering the family as a care unit. When a family is faced with a sick loved one, it requires them to reorient their lives. Although it is recognized that the family has a great capacity to adapt to social, political, cultural and other changes, a situation of illness is an event that can potentially generate a crisis for the family. In this sense, the Community Nursing Specialist in the area of Family Health Nursing is the one who ensures care in partnership, empowering the family based on their health potential. Therefore, with the aim of developing specific and common skills, it was decided to carry out the Professional Internship in a family health unit in the North region. It should be noted that in the area of the development of specific skills, clinical practice was based on compliance with the skills defined by the Portuguese Nursing Association, established in the Regulation of Specific Skills of the Specialist Nurse in Family Health, published in the Official Gazette, Series II, No. 135 of July 16, 2018, namely: caring for the family as a care unit, and for each of its members throughout the life cycle and at different levels of prevention and leading and collaborating in intervention processes, within the scope of family health nursing. In response to the training needs identified by the team, a manual to support caregivers of people with Alzheimer's was also developed, thus promoting continuous quality improvement. At the management level, we sought to have in-depth knowledge of the organization and functioning of the health unit and the health team, in order to adapt and optimize the available resources in a safe and high-quality manner. In developing these skills, we mobilized active, participatory methodological strategies using scientific evidence. The research study focused on the theme: The perspectives of families of people with Alzheimer's regarding the interventions of the nurse in family health, with the aim of knowing the perspectives of families of people with Alzheimer's regarding the interventions of the nurse in family health and with the purpose of contributing to the change/innovation of the interventions of the Nurse Specialist in Family Health with the family of a person with Alzheimer's, producing gains in Health. It is highlighted that dementia is considered as a clinical syndrome characterized by cognitive decline with a progressive and permanent character, caused by multiple etiologies, leading to Social and occupational repercussions on the sick person. Alzheimer's disease is the most common diagnosed dementia and has profound repercussions on family dynamics. A qualitative methodology was used, an exploratory and descriptive study, whose data collection involved conducting semi-structured interviews with family members of the person with Alzheimer's at home, registered at the family health unit in the northern region of Portugal. Content analysis was carried out on the findings and all ethical and moral procedures were respected. It was found that the responsibility of caring for a family member with Alzheimer's goes far beyond providing basic care, implying a complete restructuring of the caregivers' lives and requiring continuous adaptations to the growing needs of the person being cared for. The need to reconcile care with other personal and professional responsibilities often leads to significant exhaustion, resulting in feelings of isolation, frustration and, often, exhaustion. In this sense, the family health nurse must emphasize the resources that keep the family functioning according to its phase of the life cycle, establishing a therapeutic relationship with the family, helping them to find strategies to deal with the clinical situation, mitigating the exhaustion of family caregivers.
